Elderbridge is honored to announce a grant award from the Foster and Evelyn Barkema Charitable Trust on November 18, 2024. The grant provides $15,000 annually, totaling $45,000 over three years, to support our Iowa Return to Community (IRTC) program.

The IRTC program helps older adults transition from hospitals or long-term care facilities back to their homes. This funding provides critical assistance for individuals aged 60 and older whose insurance, including Medicare, does not cover essential services like safety modifications, home-delivered meals, home repairs, and medical equipment.

Your generosity fills a vital gap, supporting low to moderate-income older adults who are ineligible for Medicaid, and helping them return home safely and maintain their independence while preventing future hospital or nursing facility visits.
