Supportive Services

Supportive services are key to helping older adults maintain their long-term independence. With the help of access services (e.g., transportation, case management, information and assistance), in-home services (e.g., personal care, chore and homemaker assistance) and community services (e.g., mental health services, adult day care), individuals can prevent early institutionalization and instead continue living at home or in their community of choice.

The LifeLong Links network is an integral part of Elderbridge’s service delivery. The goals of the program are to:

  • Raise public awareness about the full range of long-term living options available in Iowa;
  • Provide objective information, advice, counseling and assistance;
  • Equip Iowans with the tools and resources they need to make informed decisions about aging in place; and
  • Connect Iowans to public and private service providers and programs at the local level.

LifeLong Links provides unbiased, reliable information and counseling to a wide range of consumers – including older Iowans – to help them obtain long-term supports and services in the most desirable and appropriate setting available.

Health, Prevention & Wellness

Health, prevention and wellness programs are critical to ensuring that older Iowans are informed about and have access to services and evidence-based activities that can significantly improve their health, function and quality of life and reduce the need for more costly medical interventions.

Benefits to Older Iowans
Studies have found that the Older Americans Act Nutrition Program promotes good health, improves nutritional status and increases socialization among participants.

Nutrition

Congregate Meals are hot, nutritious meals served to you in a community setting. Home Delivered Meals are hot, nutritious meals served to you at home if unable to visit the congregate meal site.

Falls Prevention and Evidence Based
Workshops teach you how to prevent falls and improve your overall health and well-being. Falls are a significant concern for Iowans, especially among the aging population. One out of every three adults aged 65 or older falls each year. Unintentional fall-related injuries and deaths are on the rise in Iowa and, for older adults, falls are the leading cause of injury-related deaths.

Benefits to Older Iowans

Elderbridge works to educate older adults, adults with disabilities and caregivers about a variety of evidence-based falls prevention programs available in communities across the state. These programs can help participants increase their awareness of fall hazards and improve their physical strength and balance to reduce their risk of falling.

Tai Chi is an ancient Chinese exercise program that promotes strength, flexibility and balance. This is a low-impact, relaxing form of exercise that is ideal for older individuals. Tai Chi classes are suitable for those with arthritis and other medical conditions, anyone wanting to improve balance and coordination, and/or those wanting to maintain or improve mobility.

Family Caregiver Support

Caregiver Support Groups provide you with an opportunity to connect with other family caregivers.
Training & Education provides information that will assist you in your caregiving role.
Respite Care offers temporary, substitute supports for your care recipient in order to provide you with a break.

The Family Caregiver Program works to provide family caregivers with the resources they need to enhance and extend their ability to care for a loved one.

Older Worker Employment Program

A growing number of older Iowans are choosing to join or stay in the workforce well past the traditional retirement age. But whether they are working by choice or out of economic necessity, many find the job market to be quite challenging. Older workers take twice as long as younger workers to find employment.

Benefits of the Older Worker Employment Program

Research shows that people who work into the latter half of their 60s (or beyond) benefit from a number of things, including a more secure financial future, improved access to insurance options and increased socialization.

Elder Abuse Prevention and Awareness

Elder Abuse Prevention & Awareness provides support to you if you are at-risk of or experiencing abuse, neglect, or financial exploitation. Counseling/personal assistance to help you sort through challenges and identify possible solutions.

  • Provides support and assistance if you are at risk of, or experiencing abuse, neglect, or financial exploitation
  • Referral to Adult Protective Services through the Department of Human Services
  • Coordination with community providers to improve life circumstances

Benefits to Older Iowans

This program works to provide education and develop programs and policies that can prevent elder abuse in any of its forms, including physical abuse, emotional abuse, sexual abuse or exploitation, financial exploitation, denial of critical care and self-neglect

The goal of the Elder Rights Program is to address and focus on prevention, intervention, detection and reporting elder abuse, neglect and exploitation by presenting the senior consumer with options to enhance and protect their life and independence.

Medicare Support

The Medicare Savings Program includes four separate programs.

LIS is administered by the Social Security Administration. This program helps lower Medicare Part D prescription drug costs including premiums, deductibles, and the costs of drugs on the plan’s formulary for individuals who meet the program’s income and resource eligibility requirements.

MSPs are administered by state Medicaid agencies. The program has four levels of benefits: Qualified Medicare Beneficiary (QMB), Specified Low-Income Medicare Beneficiary (SLMB), Qualifying Individual (QI), and Qualified Working Disabled Individual (QDWI). MSPs help pay the Medicare Part B premium for beneficiaries who meet the program’s eligibility requirements. If eligible for QMB, beneficiaries receive assistance to pay their Medicare Parts A/B deductibles, co-pays, and co-insurance.

Private Pay for Services

Elderbridge has a purchase of service plan that offers customized assistance and services to any adults ages 18 and above, regardless of disability or income level.
