Service Specialist for Carroll, Iowa

Case Management - Oversees client cases under the Lifelong Links Director’s guidance. - Processes client referrals and intakes as per program guidelines. - Conducts comprehensive assessments and develops service plans. - Implements and monitors service plans. - Performs at least one monthly HomeMeds assessments. - Maintains regular client contact and conducts annual reassessments or as needed. - Manages client discharges/transfers. - Records and maintains client documentation in WellSky, including intake forms, assessments, health plans, case notes, and service deliverables. - Options Counseling - Provides personalized counseling to clients. - Engages with clients to discuss goals. - Assesses needs and preferences and explains options as well as their pros and cons. - Develops action plans and identifies existing resources. - Advocates for client service decisions and identifies unmet needs. - Facilitates service access and maintains client documentation. - Provides follow-ups and conducts HomeMeds assessments. - Performs BEC Screen for Medicare-eligible clients. - Delivers presentations to community groups as requested.

Home Delivered Meals at the Sibley Senior Center

Elderbridge is looking for 1-2 people to help coordinate Congregate and Home Delivered Meals at the Sibley Senior Center. The position is currently Monday, Tuesday, and Thursday, but we would like to get back to serving Monday – Friday. Meals are catered by Carstensen’s out of Lake Park. Duties include • taking reservations for meals • working with the caterer on numbers of meals needed daily • coordinating volunteers to set up, serve, cleanup of meals • coordinate volunteers to deliver meals • counting donation money • completion of monthly paperwork • becoming ServSafe Certified Call Jody Applegate, Healthy Aging Director at 1-800-243-0678 or email at [email protected] for more information.

Advisory Council Member - Audubon, Hamilton, O’Brien, and Winnebago

What is our mission? “Our Mission is to Advocate for and Empower Older Adults, Caregivers, and Individuals with Disabilities to Pursue Independent Healthy Lives.” What is the advisory member role? The Elderbridge Advisory Council will carry out advisory functions that further the Agency's mission of developing and coordinating community-based systems of services for all older persons, caregivers, and adults with disabilities in the planning and service area. What are the Advisory Council's responsibilities? Elderbridge Advisory Council members’ dedication and leadership are vital to the agency’s mission and vision for the communities we serve. Advisory Council members should be committed to attending and participating in all advisory meetings (meetings are held quarterly in February, May, August, and October, on the 4th Wednesday of the month either via conference call or in person). In addition, the Advisory Council will exist as stipulated by the Older Americans Act, Iowa Administrative Code Chapter 17, and the Elderbridge Agency on Aging bylaws. The Advisory Council will advise the area agency on aging or matters related to older people. What is the process for becoming an Advisory Council member? Each candidate will receive an application, bylaws, and job description to complete and submit to the Board Governance Committee. Your application and credentials will be reviewed before submitting a recommendation to the full Board.
