Family Caregiver Options Counseling
Through the Family Caregiver Options Counseling program, trained service specialists can connect you to information and resources through home and community-based service providers.
Caregiver Specialist will:
• Listen to your requests or problems.
• Answer your questions.
• Give you choices of service providers in your area (ex. respite care, home health care, homemaker services).
• Make referrals if you wish.
• Assist with applications (ex. Medicaid, food assistance, Medicare Savings Programs).
Family Caregiver Case Management
• Through the Family Caregiver Case Management program trained services specialists can connect you to information and resources through both local and community-based service providers.
• Caregiver Specialist will-
• Listen to your requests or problem.
• Answer your questions.
• Give you choices of service providers in your area (ex. respite care, home health care, homemaker services). And assist in setting up those services
• Make referrals if you wish.
• Assist with applications (ex. Medicaid, food assistance, Medicare Savings Programs).
Family Caregiver Case Management FAQ’s
• How long is the program?–Over 90 days, or long term
• How do I know if I qualify?-Anyone 18 and older, who cares for someone who is 60+ OR diagnosed with a form of Dementia and does not get paid for caregiving.
• Do I have to live in Iowa?-No. As the caregiver you may live anywhere, but the care recipient must live in our 29-county service area.
• Do I have to provide daily care to qualify?-No, caregivers may provide day-to-day care in the home, or remote care in the home.
Grandparent and Older Relative Caregiver
Through the Grandparent and Older Relative Caregiver program trained service specialist will work to coordinate resources and services to support the caregiver in their role.
Grandparent and Older Relative Caregiver FAQ’s
• How long is the program?-The program is 3 months, but you may be discharged sooner depending on your specific needs.
• How do I know if I qualify?–Must be a grandparent or step-grandparent of a child; or a relative of a child by blood, marriage, or adoption who is not a parent and who is 55 years of age or older and:
1. Lives with the child; and
2. Is the primary caregiver of the child because the biological or adoptive parent is unable or unwilling to serve as the primary caregiver of the child; and
3. Has a legal relationship to the child, such as legal custody or guardianship, or is raising the child informally.
